Skilled Nursing / Long-term Care

Quality of life and quality of care go hand-in-hand at our community, where our skilled nursing and long-term care services deliver holistic care with respect for an individual’s unique needs.

Post-Acute Care / Short-term Rehab

Our community is known for providing exceptional care. Our professional and dedicated staff provides occupational, physical and speech therapy on a daily full-time basis. Treatment programs are designed to meet the physical and emotional goals necessary for residents to reach their optimum levels of functioning.  A warm welcome and creative approach in treatment is the core of the motivating spirit that is generated within the Rehabilitation Department.  

Advanced Care Unit

We know that memory loss can be challenging and frightening for the individual and their loved ones. Our Memory Care provides assistance and support for residents struggling with cognitive impairment. Memory support services provide specific assistance and emotional support for residents and their families. Services include fun and intellectual brain fitness activities that focus on a holistic approach to wellness.

Respite Care

St. Patrick’s understands that being a caregiver is a loving and difficult job. We also recognize the importance of taking care of yourself, and that doing so can often be a challenge. We want you to know we are here, to provide personalized care for your loved one in a comfortable and secure environment; so that family members and/or caregivers can enjoy a much-deserved break or vacation.

Hospice / End of Life Care

Our approach to residents' care is based on the belief that they come to us with a history and values that extend beyond their current medical needs. As we provide care, the dignity of each person is our top priority.
